A Fishbone diagram (or Ishikawa diagram) is a tool often used in lean an quality management for helping articulate route causes analysis.

It’s commonly used to help in defect analysis when you need to brainstorm possible causes of an issue. (click the link to find out more about fishbone diagram).

The diagram is structured around an issue – that runs through the center line – route causes are shown as spikes running from the centre and sub clauses are associated with the route cause lines.

Creating a fishbone diagram is fairly easy – you can create one using Microsoft Excel in no time at all.

1/ Create a new workbook – turn off gridlines and add in the drawing toolbar
2/ Use the line tool and arrow tool to create the basic framework
3/ Use the text box tool to create titles for the route causes
4/ Use a combination of text boxes and lines to create sub-clauses

    Here is another version of an fishbone template in excel. Simply fill out the table and the diagram populates itself. There are some limitations to this template but I’ve found it useful.
